Rename unique form

In the Study Designer application, you can design unique forms for study visits. A unique form is a CRF template that serves as a starting point for creating new forms for different visits of the clinical trial. Unique forms are stored in a separate folder under the CRF DESIGN tab and serve as a reusable blueprint for creating other CRFs in the study, cycle, or folder visits.

After adding unique forms to your study, it may occur that some of them need to be renamed due to new protocol requirements or mistakes in the naming convention. When renaming a unique form, all of the corresponding visit forms linked to it become updated automatically preserving the latest name changes.

To rename a unique form
  1. In the Study Designer application header, select the CRF DESIGN tab.

  2. In the toolbar, select the latest draft CRF version marked with the respective unlock symbol unlocked_icon_purple.png, otherwise, you cannot perform any actions.

  3. In the left pane that appears, expand the Unique Form folder and right-click the needed form.

  4. In the context menu that opens, select Rename pencil_icon.png.

  5. In the Rename dialog that appears, update the unique form name. When updating the name, it automatically propagates to all of its linked visit forms.

    Important

    Switch the Rename for all CRF Forms toggle on if you want to name visit forms linked to this unique form differently. Then expand each visit to enter a new name for the corresponding forms. Note that even after naming the linked forms differently, the linkage to the unique form preserves.

  6. Select save_button_red_white.png to update the unique form name.

Once saved, a unique form name is updated.
