Grid form
Grid Form comprises rows and columns. It can be used to acquire data for the kind of questions that require multiple answers for different fields of the same question.
For example, the question "Enter the total of subjects participating since last visit" can provide the following fields: Screened, Screen Failure, Ongoing, Enrolled, Completed, Early Exit. In each field, the reviewer is expected to enter a value. Therefore, for such questions, a grid form is created.
