Delete simple or log form

In CTMS, you can delete a simple or log form from the monitor template. This may be required if the study protocol requirements have changed, the site or study requires a different form, the data in the form is wrong or duplicate, and so on.

To delete a simple or log form
  1. In the CTMS application header, select the LIBRARY tab.

  2. On the page that opens, from the left pane, select the Monitor Template subtab.

  3. From the Monitor Template List table, select the template name in which the required simple or log form exists.

  4. On the Monitor Template Detail page that appears, from the explorer pane, locate the required simple or log form, and right-click it.

  5. From the dropdown menu that appears, select Delete trash_can_icon.png.

  6. In the Delete Item dialog that appears, confirm the action by selecting delete_button_white_red.png.

Upon confirmation, the simple or log form is deleted from the monitor template.
