Add data to monitor template

The Monitor Template subtab provides you with the ability to create 3 types of forms: Simple Form, Log Form, and Grid Form. After you have created a monitor template, you can start adding and creating any type of form required for your monitoring visit. Once you create a form, you can then add data (questions) to the forms using various editing tools provided.
